Home Health Aide; Per Diem, Minimum 24 hours/Month!

Job Summary

The Home Health Aide provides personal care and light homemaking services to home care patients in accordance with the plan established by the primary nurse/therapist in each case. The Home Health Aide reports to the Rehabilitation Supervisor, Home Care.

Minimum Qualifications

• Education
  • High School diploma and successful completion of the training program required by CMS or the equivalent.
• Experience
  • Preferred
• Licensure and/or Certification
  • Home Health Aide or CNA certification preferred; BLS for the healthcare provider.
• Skills

  • Organizational skills
  • Basic computer skills
  • Ability to maintain confidential information
  • Ability to read, write, and communicate in English
  • Excellent customer service skills
  • Valid Driver’s license and ability to use own vehicle for transportation.
