Provides leadership and oversight of the Quality Improvement Program and collaborates with the quality improvement staff in executing the quality improvement program. Leads and directs process and overall quality improvement activities that produce better patient care and more efficient operations.
Education: „X Graduate of a Nursing, Therapy, or Health Administration Program required. Bachelor¡¦s degree in an associated program required.
Experience: 5 years¡¦ experience in a home health setting.
Previous experience as a QI Coordinator or Manager in a home care setting preferred.„X
Previous experience with OASIS and Coding.
Past experience working with SHP or other similar program.
Licensure and/or Certification: Current Massachusetts Registered Nurse License required.
Competent in current homecare technology to include computer use and work, excel, and PowerPoint programs.
Previous use in Netsmart preferred.„X
Must be able to read, write, and communicate in English. „X
Ability to develop plans as they relate to quality improvement. „X
Ability to assess, plan, implement, and evaluate programs and data. „X
Must be able to demonstrate competency in leadership skills, in learning/facilitation, and interpersonal communication skills.
Must be a dynamic, motivated, and creative self-starter, who continuously keeps abreast of current trends in Performance and Quality Improvement and with own professional growth. „X
Must be outcome focused with a focus on quality. „X
